COVID-19 Screener
On the day before your day of service, you must submit a response to the COVID-19 screener which you have received via text message or email. If you have any questions about the process, click here to view the frequently asked questions below.
Clocking in and Clocking Out
You must verify your hours of work in one of three ways:

Clocking in and clocking out via the Santrax Mobile App (Mobile Visit Verification)

Dial to call Santrax to complete clocking in and clocking out using your client's phone (Electronic Visit Verification)
Task Verification
The task verification form is used to keep track of the tasks you complete for your client (e.g. cleaning or meal preparation). You need to fill out the task verification form every time you visit and have your client sign it.
Money Documentation
The money documentation forms keep track of any money exchanges you may need to do for your client (e.g. getting groceries or running errands). You need to fill out the money documentation forms each time you exchange money with your client. The form has two parts, be sure to fill out both and have your client sign them.
