The following lists display documentation that is needed from new and existing home care aides. Please find the documents that you are required to fill out and use the links to access them.
New Employees

Policy Manual Acknowledgement

All MedTec staff are required to acknowledge that they have access to and have reviewed the contents of the MedTec Policy Handbook
Emergency Contact

All MedTec staff are required to have an emergency contact on file
Diploma / Work Verification form
-
All home care aides are required to submit a diploma (high school or beyond) or a work verification form to show previous homecare experience
Fingerprinting Request Form
-
All home care aides are required to conduct fingerprinting to complete background checks for employment. Fingerprinting will require a valid ID and documentation from MedTec to bring to a fingerprinting facility
HIPAA Agreement
-
All MedTec staff are required to acknowledge that they have reviewed the contents of the MedTec Policy Handbook’s section on Health Information Privacy and agree to its terms
Marketing Agreement
-
All MedTec staff are required to acknowledge that they have reviewed the contents of MedTec’s policy on employee restrictions on marketing and agree to its terms
Signed Job Description
-
All MedTec homemakers are required to acknowledge that they have reviewed the description of their position and agree to the terms and conditions of their employment
Drive / No Drive Agreement

All MedTec home care aides are required to disclose whether or not they intend to drive on behalf of the company and if so, required to submit a valid driver’s license and proof of valid auto insurance
Direct Deposit / Voided Check
-
MedTec home care aides must set up a direct deposit account in order to receive income and to be eligible for payroll
Communication Consent

All MedTec home care aides must give consent to be contacted and scheduled for their ongoing work for different clients
W-4 Submission
-
All MedTec home care aides must submit a W-4 tax statement to be eligible for payroll
Background Check

All MedTec staff are required to submit information necessary for background checks prior to employment
I-9 Employment Authorization and ID

All MedTec staff are required to submit information necessary to conduct an I-9 employment authorization check along with a scanned copy of a valid form of identification

Forms that are required daily:
Task Documentation

All MedTec home care aides are required to submit a summary of the tasks completed during their service for a client each week
Money Handling
-
All MedTec home care aides are strongly advised to log any financial transactions using client money with MedTec records for the purposes of their own protection from claims of theft or improper money-handling. Please ensure to complete part one and part two
Forms that are required on an ongoing basis
Quarterly Conference
-
All MedTec home care aides are required to provide feedback on their ability to fulfill their client's plan of care on a quarterly basis. These conference forms are due March 15, June 15, September 15, and December 15 of every year
Driving Policy
-
All MedTec home care aides are required to disclose whether or not they intend to drive or accompany clients in a motorized vehicle and submit requisite documentation. An updated response to the Driving Policy is due on June 15 and December 15 of every year for all homemakers who accompany or transport clients in motorized vehicles
Influenza Vaccine Form

All MedTec home care aides are required to submit an Influenza Vaccine Form every year indicating whether or not they have been vaccinated. Vaccination Forms are due by January 31 every year
Additional Hours Request Form
-
MedTec home care aides may request additional hours of work depending on their availability via submission of a request form
